This team works with people with multiple long term health conditions or older people who need extra support to live independently.

Care Navigators are part of the NHS. They are here to help people with more complex health and care needs, such as people who have just been discharged from hospital or who are unable to leave their home.

Social Prescribing Link Workers (sometimes called SPLWs or Social Prescribers) work in GP practices and are part of the NHS.

SPLWs are here to help you to connect with community activities, groups and services to support the practice, social and emotional needs that affect your health and wellbeing, such as housing and money.

The SPLW will work with you over a fixed number of appointments to help you to achieve your health and wellbeing goals.

Keeping active can help you have a healthy weight and reduce your risk of health problems like heart disease, stroke and dementia. It also helps you feel better, happier and less stressed.

A stroke happens when blood supply to a part of the brain is cut off. The sooner a person can get medical help, the less damage the stroke is likely to have.

If you think someone is having a stroke, call 999 immediately and ask for an ambulance.
