Social Prescribing Link Workers (sometimes called SPLWs or Social Prescribers) work in GP practices and are part of the NHS. 

SPLWs are here to help you to connect with community activities, groups and services to support the practice, social and emotional needs that affect your health and wellbeing, such as housing and money. 

The SPLW will work with you over a fixed number of appointments to help you to achieve your health and wellbeing goals.

Keeping active can help you have a healthy weight and reduce your risk of health problems like heart disease, stroke and dementia. It also helps you feel better, happier and less stressed. 

We are in a world where most things you need to do are now online. If you can use a computer, browse the internet and fill out online forms, this will help you to be able to do more practical things on your own. 

The Well Newham Directory of Services (powered by Joy) is a directory of all the different health and wellbeing services you can access in Newham. 

There are over 500 Newham-based services listed on the platform. From stop smoking services, to free nutrition courses and social events near you, it's now really easy to find out what's happening in your community.
