Care Coordinators

Care Coordinators are based in GP practices and are part of the NHS.

This team works with people with multiple long term health conditions or older people who need extra support to live independently. 

What do Care Coordinators do?

Care Coordinators support residents with multiple long term health conditions or older people who need extra support to live independently. 

They can support you to:

  • Prepare for your doctors and hospital appointments
  • Develop a personalised care plan
  • Follow up with you afterwards to see how the appointments went
  • Connect you into other local services that could help you

Care Coordinators offer home visits.

We take your privacy very seriously. As part of our service, we use the Joy platform to store your personal data so we can keep up-to-date records about your health and wellbeing in a safe and secure way. 
 
We will not share your data with anyone or any service without your consent. If you decide you do not want your data stored on the Joy platform, you can tell us and we can delete the data permanently.
