Dementia Support in Newham

Information: The Memory Clinic assesses people who have had memory problems which might be the result of dementia. The team provides yearly follow ups and can prescribe medication and refer to support.

Eligibility:

· Resident of Newham

· Memory problems for more than six months

· No untreated health issues that affect memory

· Not living in a care home

Information: This team supports older adults with complex psychological needs, including dementia. They assess needs, diagnose, provide psychological support, operate group activities, manage medication and work with care homes.

· Older adults with complex psychological needs including people presenting with dementia who have additional health needs

· Usually only supports age 65+ but may support people with early onset dementia or frailty

Information: Social Care provides assessments and support under the Care Act. This can include equipment, day services, home care and safeguarding support.

Information: The Newham Admiral Nurse is specialised in dementia. They support families with emotional and practical challenges such as communication difficulties, changes in behaviour and long term planning.

· Not living in a care home or receiving regular support from an NHS Community Mental Health Team

· Receiving input from a Community Health Service: District Nurses, Referral and Assessment Team (RAT), Urgent Care Response (UCR), Occupational Therapy (OT), Physio, Rehab Support or Speech and Language Therapists (SALT)

Moments of Joy hosts a weekly social club called “Lemonade Days” with food, music and creative activities that help people enjoy positive experiences together. These run every Tuesday 11-2:30 pm in St Mark’s Community Centre in Beckton (218 Tollgate Road). Self-payment of £10 per person / session.

English National Ballet offers dance sessions in Canning Town for people living with dementia. Sessions include gentle movement, live music and time to socialise with others. Classes are designed for all abilities and aim to support wellbeing, confidence and connection through creative activity. Self-payment of £40 per person for a 10-week term.
