Annual health checks

If you are over 14 and have a learning disability, you should get a health check from your GP every year.

If there's anything the doctor thinks you could do to be healthier, they will make a plan with you with some ideas on what you can do to be healthier.

Hospital passports

A hospital passport tells the hospital about you (your learning disability, your health, your likes and dislikes and how you like to communicate).

When you're in hospital, you can show it to doctors and nurses so they know all about you and how to make things easier for you.
