The Armed Forces Covenant is a promise on behalf of the UK that people who serve or have served in the Armed Forces, and their families, are treated fairly.

AskSARA is our new online guided advice tool that is here to help residents find useful information about products to make daily living activities easier.
 
The platform will provide residents with impartial advice about suitable equipment and assistive technology solutions to help residents of all ages to live independently and maintain their lifestyle.

Care Coordinators are based in GP practices and are part of the NHS.

This team works with people with multiple long term health conditions or older people who need extra support to live independently. 

Community Neighbourhood Link Workers work as part of Newham Council's Adult Social Care team.

They are here to support people who are lonely and / or isolated to find and access services in the community to improve their wellbeing.

The Community Connectors programme is in partnership with Community Links, Aston Mansfield, MindTHNR and East London NHS Foundation Trust (ELFT).

We are committed to supporting people in Newham who are open to a mental health team to thrive within their communities.

Speak to your mental health professional to get a referral to the Community Connectors.

Social Prescribing Link Workers (sometimes called SPLWs or Social Prescribers) work in GP practices and are part of the NHS. 

SPLWs are here to help you to connect with community activities, groups and services to support the practice, social and emotional needs that affect your health and wellbeing, such as housing and money. 

The SPLW will work with you over a fixed number of appointments to help you to achieve your health and wellbeing goals.
