Grief and bereavement

Care Coordinators are based in GP practices and are part of the NHS.

This team works with people with multiple long term health conditions or older people who need extra support to live independently. 

Community Neighbourhood Link Workers work as part of Newham Council's Adult Social Care team.

They are here to support people who are lonely and / or isolated to find and access services in the community to improve their wellbeing.

Social Prescribing Link Workers (sometimes called SPLWs or Social Prescribers) work in GP practices and are part of the NHS. 

SPLWs are here to help you to connect with community activities, groups and services to support the practice, social and emotional needs that affect your health and wellbeing, such as housing and money. 

The SPLW will work with you over a fixed number of appointments to help you to achieve your health and wellbeing goals.

We all go through times when we feel worried, confused or down. Mental health problems are when difficulties get in the way of how we think and feel. This can mean we find it hard to cope with family life, relationships, school, work or the wider world.

Planning for end of life is really important.

Using the MyWishes platform is a great way to make sure your wishes are written down for your family to access.

A pre-paid Funeral Plan allows you to pay upfront or in instalments for a funeral. This may help relieve the financial burden on family and protect them against rising funeral costs.

Coping with the loss of a loved one is one of the most difficult things to happen in our lives.

During times of grief, there is somebody you can talk to and organisations that offer a range of support.
